Off-Campus Education/Intake Request

Per our new member intake and education guidelines, all intake and new member education activities are to take place on the UConn campus. In recognition that some organizations have activities and rituals that require being off-campus, an organization can request permission from the CFSD Director for off-campus activities. This form must be submitted two weeks prior to the requested off-campus activity for review.

To check the status of a previous submission,  please log in to uconn.kualibuild.com, then select "my documents" on the top center portion of the screen. This will populate a list of all documents submitted through this platform. Select the correct submission from the list. This will open a new tab of the submission. On the top right corner, you will find three ellipses - once you click on this, you will see the "workflow status" option.  This will show you where in the review process your form currently is.
