Annual Compliance Requirements

Beginning Fall 2016, the Office of Fraternity and Sorority Life requires that all recognized fraternities and sororities submit the following documents by the first Friday of the Fall semester:

  • Certificate of Insurance
  • Risk Management Policy
  • Crisis Management Policy (if applicable)

If at any point during the academic year any of the above changes or expires, the organization must submit an updated version within five (5) business days.

Annual Compliance Document Submission

  • Documents

The following documents are needed to ensure organization compliance with University expectations, and to assist University staff in properly advising and supporting the Chapter. All documents submitted will be accessible and viewable by University staff and affiliated members of your organization only. Other students, including OFSL student staff, will NOT have access to this information.
  • Please upload evidence of a comprehensive general liability insurance policy so as to “save harmless” the University, with coverage of not less than $1,000,000 per occurrence / $2,000,000 aggregate. The certificate shall name the University as an additional insured.
